Diabetes Management

Both our GP’s and Nurse’s are actively involved in your diabetes management. We can provide training in glucose monitoring and commencing insulin as well as providing advice on preventative measures such as diet & exercise. We will coordinate your care with other members of your multidisciplinary team and organise referrals as needed.

Sr Elaine Cornford is an accredited Diabetes Educator - available for appointments weekdays, Phone reception to book in. You will need an EPC referral from your GP to make an appointment. 

 

We will send you a reminder letter when you are due for a check-up and monitor your other health conditions as well.
