Referrals, Medical Certificates, Forms that need filling in

 

Please make an appointment with your usual GP for these items. They will not be provided otherwise.

 

  • Forms (NDIS, Drivers License, Mobility Parking, Carers forms, medication charts, etc)

Do not bring forms in to the reception counter and ask us to ‘get the doctor to fill in and I’ll pick them up another day’. The doctor is unlikely to have time to complete your request which is why we ask that you make an appointment otherwise forms left lying around on the doctors desk could get misplaced or destroyed.

 

  • Medical certificates

We do not leave out medical certificates for work/school/carer’s leave etc. You need to make an appointment for a Medical Certificate – even if you feel you/they weren’t sick enough to need to see a doctor but you just need it for work/school/daycare – that’s their policy, this is ours. Alternatively most pharmacists will provide a medical certificate however you still need to go there and see them, and they are likely to charge a fee. Medical Certificates cannot be back-dated.

  • WorkCover Certificates of Capacity

WorkCover certificates usually last for about a month - Your workcover certificate has the dates that certificate is valid to so you have more than enough time to book your next review appointment. We recommend making your next appointment before you leave.

If you let your certificate lapse your insurance company may put a hold on your payments until you can provide an updated certificate and we are not able to provide one without an appointment with your regular GP - which could be a couple of days or a couple of weeks!

 

  • Lost referrals

In some cases, If your doctor has saved a copy of your referral we are able to reprint for pick-up or alternativley we can email a copy of your referral - at our earliest convenience. Please be patient has we have many enquiries to process each day. 

 

• New Referrals

If you are requesting a referral somewhere that your GP has not referred you to previously, you need to make an appointmen to obtain the referral and discuss your reasons for requesting the referral.
